Progressive renal regardless of the initiating insult, is characterized by glomerulosclerosis, tubulointerstitial fibrosis and vascular sclerosis. The severity of the tubulointerstitial fibrosis closely parallels the decline in renal function. Primary mechanisms of progressive scarring of the tubulointerstitium include up-regulated tubular expression of cytokines and extracellular matrix, adverse consequences of infiltrating macrophages, and possible epithelial-mesenchymal transdifferentiation, resulting in a profibrotic interstitial infiltrate.
