Aim: This review aims to assess current strategies for managing severe osteophlegmons, focusing on effective and safe treatment options. It provides an overview of both surgical and pharmacological therapies and their integration for optimal clinical outcomes. Materials and Methods: A literature search was performed using “perimandibular space infection” in Scopus, Web of Science (WOS), and PubMed and 5 final articles were analized. Conclusions: Osteophlegmons pose serious risks, including osteomyelitis, sepsis, and mortality. Effective management requires early diagnosis and a multidisciplinary approach combining surgical intervention and pharmacological therapy. Surgical treatment, including debridement and drainage, helps control infection locally, while antibiotics are crucial for eradicating pathogens. A multimodal approach with early surgery and antibiotics significantly improves outcomes and reduces complications. Further randomized controlled trials with larger sample sizes are needed to refine guidelines for managing these complex infections.
