Surgeryforcongenitalvalveanomalies inchildrenisachallengingtopic.Weaim to assessearlyandlatefunctionaloutcomesofCorMatrixscaffoldafterrepairof aortic andpulmonaryvalves(PV)incongenitalheartdiseaseinaprospective nonrandomizedclinicalstudyonchildren withcongenitalaortic(Group1)orPV (Group 2)disease.Primaryendpointswere reoperationorreinterventionon semilunarvalvesandechocardiographic evidenceofregurgitationorstenosis greater thanmild.ResultsofPVrepairin tetralogyofFallotwerecomparedwitha control groupofpatientswhounderwent PVrepairwithpolytetrafluoroethylene. A totalof22consecutiveselectedpatientswithcomplexcongenitalheart diseasewereincluded:PVrepairin18and aorticvalverepairin4.Atdischarge bidimensional echocardiography,semilunarvalveregurgitationwasmildin50% of allpatients.Atamedianfollow-upof 23months(4.3-51.3),reoperationfor aorticvalvereplacementwasnecessaryin2;rightventricularoutflow tract obstructionoccurredin3,requiringinterventionaltreatmentin2.Pulmonary regurgitation degreeworsenedinmostpatientsofGroup2atfollowup.When comparing patientswithtetralogyofFallotinGroup2withpatientswho underwent PVrepairwithpolytetrafluoroethylene,therewerenosignificant differences inPVfunctionatfollowup.We concludethatCorMatrixscaffoldfor semilunarvalvereconstructiondoesnotpresentwithsignificantadvantagesto traditional materialsatmidtermfollowup.Inaddition,whenusedforpulmonary valvereconstruction,CorMatrixseemstoshowgradualfunctionaldeterioration in themidterm.
