The pandemic emergency has highlighted numerous and important flaws in the organization of the provision of primary care and care actions, despite the ability to react already demonstrated since the beginning of the epidemic. The flaws do not exclusively concern organizational aspects, but are rooted in the difficulty, above all of a cultural and political nature, of intertwining health and social, individual and community, institutional and interinstitutional dimensions in an effective, productive and protective way. In particular, and without claiming exclusivity, we draw attention to the obvious failure of interventions centered on the "patient" understood as predominantly "sick body", and not as a "person with needs"; on the delays and resistances to a complete integration between hospital and territorial medicine; on the concern of the response of primary care to the needs of patients, and this not by the will of the actors, but by the insufficient implementation of health policy guidelines present in our system since law 833/78.
