A variety of excisional procedures have been described for advanced lymphedema. Charles’ Procedure is a surgical treatment of limb lymphedema. In this procedure, the lymphedematous tissue is radically excised, and the limb is covered with skin grafts harvested from the surgical specimen. Even though successful outcomes after Charles’ procedure have been reported, potential complications of the procedure are the recurrence of the lymphedema especially at the foot necessitating regrafting and toes amputation, and poor cosmetic result. To prevent this complication, the modified Charles’ procedure with lymph node flap is described in the advanced lymphedema.
