Spinal cord vascular lesions are rare entities, whose diagnosis, classification, and treatment pose specific problems. An explicit classification has been recently proposed by Spetzler et al, based on anatomic and pathophysiologic features, which may suitably guide treatment. Clinical manifestation and evolution depends on the underlying pathophysiologic mechanism: hemorrhage, spinal cord ischemia related to either steal of spinal cord blood flow or to venous congestion or venous thrombosis, and mass effect. Whether extra- or intramedullary, symptomatic spinal vascular lesions are frequently managed by endovascular techniques exclusively or in association with surgery. In this chapter, we focus on the endovascular treatment of arteriovenous fistulas (AVFs) and arteriovenous malformations (AVMs), which are most amenable to endovascular treatment. Tumoral lesions such as hemangioblastomas and cavernomas are not included in this chapter as they are mostly approached by surgery alone.
